Welcome to our office. We are honored that you have chosen us as your ophthalmologists. We strive to provide the highest quality care for all of our patients in a timely and respectful manner.
New patient appointments usually take 2 hours. As a part of your thorough exam, your eyes will be dilated. we advise you to bring a driver with you.
Please bring with you your insurance card, a form of identification, your list of health problems, medications, and allergies.
For your convenience, you can print and fill out the forms below prior to your first visit, and bring them with you to the office.
